One Name, Two Different Nutrients
Most vitamins are a single compound or a tight family of closely related ones. Vitamin A is not. “Vitamin A” is an umbrella term covering two chemically distinct groups that your body handles in completely different ways — and confusing them is behind almost every piece of bad vitamin A advice in circulation.
Preformed vitamin A (retinoids). Retinol, retinal, and retinyl esters like retinyl palmitate and retinyl acetate. These come from animal foods — liver, egg yolk, dairy, fish — and from many supplements. They are ready to use, absorbed efficiently, and stored in the liver. Because storage has no automatic shut-off valve, sustained high intake accumulates.
Provitamin A carotenoids. Beta-carotene, alpha-carotene, and beta-cryptoxanthin, from orange, yellow, and dark green plants — carrots, sweet potato, pumpkin, spinach, kale. Your body converts these to retinol as needed, and the conversion is regulated: when you have plenty, conversion downshifts. This is why you cannot get vitamin A toxicity from carrots (you can get carotenemia, a harmless orange tint to the skin, which resolves when intake drops).

Reading the Units: RAE, IU, and Why Labels Confuse People
Vitamin A is measured in micrograms of retinol activity equivalents (mcg RAE), a unit designed to account for the fact that carotenoids convert inefficiently. Roughly:
- 1 mcg RAE = 1 mcg retinol
- 1 mcg RAE = 12 mcg dietary beta-carotene
- 1 mcg RAE = 2 mcg supplemental beta-carotene (better absorbed than the food form)
Older labels use International Units (IU), which don’t distinguish between forms and therefore mislead. As a rough conversion, 1 IU retinol ≈ 0.3 mcg RAE, while 1 IU of beta-carotene is worth far less. A “10,000 IU vitamin A” label means something very different depending on whether it’s retinyl palmitate or beta-carotene — and that’s exactly the distinction the IU unit hides. What Vitamin A Actually Does
- Vision. This is the role with the tightest, most direct biochemistry. Retinal combines with the protein opsin to form rhodopsin, the light-sensitive pigment in the retina’s rod cells that makes low-light vision possible. Impaired night vision is one of the earliest signs of genuine deficiency. Note that vitamin A’s eye role is distinct from that of lutein and zeaxanthin, which are non-provitamin carotenoids concentrated in the macula — the broader picture is in our eye health roundup.
- Immune function. Vitamin A maintains the integrity of epithelial barriers — the linings of your respiratory tract, gut, and eyes — which are the immune system’s first physical defense. It also participates in the development and function of several immune cell types. It supports normal immune function; it does not “boost immunity.”
- Cell growth and differentiation. Retinoic acid acts on gene expression, directing cells toward their mature specialized forms. This is why vitamin A derivatives are used pharmaceutically in dermatology, and also why excess is a developmental risk in pregnancy.
- Skin. Related to the above. Topical and prescription retinoids are well-established dermatologically, but that is not an argument for oral megadoses — the prescription route delivers targeted drug therapy with medical monitoring.
How Much You Need
- Adult RDA: about 900 mcg RAE/day for men, 700 mcg RAE/day for women.
- Pregnancy: about 770 mcg RAE/day. Nursing: about 1,300 mcg RAE/day.
- Tolerable upper intake limit: 3,000 mcg RAE/day from preformed vitamin A. The limit applies to retinoids, not to provitamin A carotenoids.
Deficiency is rare in populations with reliable food access, though it remains a serious public health problem globally and is a leading preventable cause of childhood blindness worldwide. In well-fed populations, meaningful shortfall shows up mainly in people with fat malabsorption — cystic fibrosis, Crohn’s disease, celiac disease, chronic pancreatic insufficiency, or after certain bariatric procedures — because a fat-soluble vitamin needs functioning fat absorption to be taken up at all. A relevant interaction: zinc is required to mobilize vitamin A from liver stores and to produce retinol-binding protein. Zinc deficiency can therefore produce functional vitamin A shortfall even when liver stores are adequate.
The Safety Section — the Important Part
Vitamin A is one of the few supplements where “more” carries real, documented downside.
Chronic excess of preformed vitamin A (hypervitaminosis A) can cause headache, bone and joint pain, dry or peeling skin, hair loss, and liver damage; long-term high intake has also been associated with reduced bone mineral density and higher fracture risk in observational research. Acute massive doses cause a distinct, more dramatic picture. Neither comes from food carotenoids — the culprits are high-dose supplements, cod liver oil taken generously, and frequent liver consumption (a single serving of beef liver can exceed the daily upper limit several times over).
Pregnancy is the sharpest warning. Excess preformed vitamin A is a recognized teratogen — high intake during pregnancy is associated with birth defects. Practical implications: pregnant people should avoid high-dose retinol supplements, avoid liver and liver products, and check that any multivitamin delivers its vitamin A mainly as beta-carotene. Beta-carotene supplements and smokers. This is the exception to “carotenoids are harmless.” Large trials of high-dose beta-carotene supplements found an increased incidence of lung cancer in smokers and asbestos-exposed workers — a result striking enough that trials were stopped early. The signal applies to high-dose supplements, not to beta-carotene from food. Current smokers and former smokers should avoid high-dose beta-carotene supplements.
Medication interactions:
- Oral retinoid drugs (isotretinoin, acitretin) — combining these with vitamin A supplements stacks toxicity risk. Don’t.
- Anticoagulants — very high vitamin A intake may increase bleeding risk.
- Orlistat and bile acid sequestrants reduce fat-soluble vitamin absorption.
- Liver disease or high alcohol intake — the liver both stores and metabolizes vitamin A, and toxicity risk is elevated. Supplement only under medical guidance.
Practical Use
If you and a clinician decide supplementation makes sense: take it with a meal containing fat for absorption, prefer products that specify the form, and favor beta-carotene or a mixed formulation over high-dose retinyl palmitate unless there’s a specific clinical reason. Check whether your multivitamin already supplies a full day’s worth — stacking is how people quietly cross the upper limit. Vitamin A is also worth thinking about alongside the other fat-soluble vitamins, since they share absorption pathways and storage behavior — vitamin D3 in particular is far more commonly supplemented and far more commonly deficient.
Bottom Line
Vitamin A is essential and easy to get from food, and for most people supplementation isn’t necessary. If you do supplement, the form on the label matters more than the number: preformed retinol accumulates in the liver and has a real upper limit of 3,000 mcg RAE/day with serious pregnancy implications, while plant beta-carotene converts on demand and is safe from food — with the notable exception of high-dose beta-carotene supplements in smokers. When in doubt, get vitamin A from carrots, sweet potato, and leafy greens rather than a bottle.
