Methylation Explained: The Nutrients Behind Your Body's Busiest Chemical Reaction

One methyl group, a billion reactions a second, and a whole industry of overselling.

Search “methylation” and you’ll land in one of two worlds. In the first, it’s a quiet piece of undergraduate biochemistry — the transfer of a single carbon atom with three hydrogens attached. In the second, it’s the master key to your energy, mood, hormones, detoxification, and aging, unlockable for $79 a month if you buy the right methylated B-complex.

The first world is accurate. The second borrows its vocabulary. Here’s what’s actually going on, and what genuinely matters for what you eat and take.

What Methylation Is

A methyl group is a tiny chemical tag: one carbon, three hydrogens. Methylation is simply the act of attaching one to another molecule — and demethylation is taking it off. Your cells do this constantly, an estimated billions of times per second, and the tag acts like a switch or a label that changes what the receiving molecule does.

Reactions that depend on it include:

  • Neurotransmitter turnover — building and breaking down serotonin, dopamine, adrenaline, and melatonin
  • Gene expression — methyl tags on DNA help determine which genes are read and which stay quiet
  • Membrane building — converting phosphatidylethanolamine to phosphatidylcholine
  • Creatine synthesis — one of the single biggest consumers of methyl groups in the body
  • Clearance of hormones and other compounds — a component of what marketing loosely calls “detox”
  • Recycling homocysteine back into the amino acid methionine

That last one is where the nutrition comes in.

The Cycle, Without the Jargon

Methionine, an amino acid you get from dietary protein, is converted into SAM (S-adenosylmethionine) — the body’s universal methyl donor. SAM hands its methyl group off to whatever needs one and becomes SAH, which is then broken down to homocysteine.

Homocysteine sits at a fork with three exits:

  1. Remethylation via folate and B12. Folate carries a methyl group to an enzyme (methionine synthase) that uses vitamin B12 as its cofactor to convert homocysteine back into methionine. This is the main route in most tissues.
  2. Remethylation via betaine. A backup pathway, active mostly in the liver and kidneys, where betaine (trimethylglycine, or TMG) donates a methyl group instead. Betaine comes from food or from choline.
  3. Transsulfuration. Homocysteine is converted toward cysteine — and eventually glutathione — in a vitamin B6-dependent step.

That single diagram explains why the same short list of nutrients keeps coming up: folate, B12, B6, riboflavin (which is the cofactor for the MTHFR enzyme that prepares folate for the methylation step), choline and betaine, plus magnesium and zinc as enzyme cofactors, and enough dietary protein to supply methionine.

Notice how ordinary that list is. Leafy greens, legumes, eggs, fish, meat, and beets cover essentially all of it.

MTHFR: Common Variant, Oversized Reputation

MTHFR is the gene for methylenetetrahydrofolate reductase, the enzyme that produces the active folate form used to remethylate homocysteine. Two common variants — usually written C677T and A1298C — reduce enzyme activity somewhat. Carrying one copy of 677T is very common, and roughly 10% of people in some populations carry two, which can drop enzyme activity to around 30-40% of typical.

Here’s the honest framing that gets skipped in the marketing:

  • The clinical consequence for most carriers is a modest rise in homocysteine, and even that largely disappears when folate and riboflavin intake are adequate.
  • Reduced enzyme activity is not the same as “you can’t process folic acid.” Nearly everyone converts ordinary folic acid and food folate perfectly well at normal intakes.
  • Major clinical bodies do not recommend routine MTHFR genotyping for the general population, and a positive result usually doesn’t change management.
  • A great many symptoms — fatigue, anxiety, brain fog — get attributed to MTHFR results by consumer testing services without any evidence that the variant caused them.

If you already know you carry a variant, the practical response is unglamorous: eat enough folate, make sure B12 and riboflavin are adequate, and if you’re worried, get homocysteine measured rather than guessing. The Homocysteine Lesson

Elevated homocysteine has been studied for decades as a marker associated with cardiovascular and cognitive outcomes. B vitamins lower it reliably — that part is not in dispute.

What happened next is one of the more instructive stories in nutrition research. Large randomized trials gave B vitamins to lower homocysteine and looked for the expected benefits. Homocysteine dropped. The clinical outcomes largely didn’t follow. That result reshaped how researchers treat surrogate markers: moving a number in the right direction doesn’t guarantee the underlying benefit was ever causal.

Keep that in mind whenever a supplement is sold on its effect on a marker rather than on how people actually do.

Sensible Intakes

  • Folate: RDA 400 mcg dietary folate equivalents for adults, higher in pregnancy. Public health guidance recommends 400 mcg/day of supplemental folic acid for anyone who could become pregnant. The upper limit applies to supplemental forms: 1,000 mcg/day.
  • Vitamin B12: RDA 2.4 mcg/day; supplements typically supply 250-1,000 mcg because absorption of large oral doses is inefficient. No upper limit is set. Absorption declines with age and with long-term use of metformin or acid-suppressing drugs — see our B12 guide.
  • Vitamin B6: RDA 1.3-1.7 mg/day; upper limit 100 mg/day. Sensory nerve symptoms have been reported with prolonged high-dose use, sometimes below that limit, so treat the 100 mg B6 in some “methylation” formulas as unnecessary risk.
  • Riboflavin (B2): RDA 1.1-1.3 mg/day. Cheap, safe, and the one most people forget in this pathway.
  • Betaine (TMG): 500-3,000 mg/day is the common supplemental range. Doses of 4 grams or more per day have been shown to raise LDL cholesterol, which is a reason not to freelance upward.
  • Choline: Adequate Intake 425 mg/day for women and 550 mg/day for men; upper limit 3,500 mg/day. Eggs and liver are the standouts.

Cautions Worth Naming

  • Don’t hide a B12 problem. High folate intake can normalize the blood picture of B12 deficiency while nerve-related effects continue. Check B12 status before taking high-dose folate long term.
  • Methotrexate works partly by antagonizing folate. Never add folate or methylfolate on your own if you take it — dosing is managed deliberately by the prescriber.
  • Anticonvulsants, levodopa, and some other medications interact with folate or B6. Bring your medication list to a pharmacist before starting a B-complex.
  • “Overmethylation” reactions — irritability, headaches, or agitation after starting high-dose methylfolate — are widely reported anecdotally and poorly characterized in research. If you feel worse on a methylation stack, that’s reason enough to stop and reassess, not to push through.
  • Pregnancy and nursing: stick to a prenatal formula and your clinician’s guidance rather than assembling your own.

Bottom Line

Methylation is real, essential, and almost entirely uninteresting as a shopping category. It runs on folate, B12, B6, riboflavin, choline, and enough protein — nutrients a decent diet or a basic multivitamin already supplies. MTHFR variants are common and usually modest; a positive consumer test result is not a diagnosis and rarely changes what you should do. If you have a specific reason to look into this — a known variant, a relevant medication, a measured homocysteine result — get the actual numbers and work from those instead of buying the stack that promises to fix a pathway you were never shown to have a problem with.
