Food First: When a Supplement Actually Fills a Real Gap

Food first is a starting rule, not a finishing argument. The useful part is knowing the exceptions.

“Food first” is the most repeated piece of advice in nutrition, and one of the least useful as stated. Taken literally it implies every nutrient is obtainable from a good diet, which isn’t true. Taken cynically it becomes a rhetorical move — dietitians use it to dismiss supplements wholesale, supplement companies use it to argue that modern food is so depleted you need their entire catalog.

The version worth holding is narrower and much more actionable: food is the default, and there is a specific, short, knowable list of exceptions. This guide is that list, plus the reasoning behind it.

Why Food Genuinely Wins Most of the Time

Three reasons, and none of them are mystical.

The matrix matters. Nutrients in food arrive packaged with fiber, fat, water, and hundreds of other compounds that shape how they’re absorbed and used. Iron from meat comes as heme, which is absorbed far more efficiently than the non-heme iron in supplements and plants. Carotenoids need dietary fat to be absorbed at all. Calcium from dairy arrives with protein and phosphorus. Isolate a nutrient from that context and you change its behavior, sometimes in ways nobody predicted.

Isolated nutrients have a mediocre track record. This is the most instructive pattern in nutrition research. Observational studies repeatedly find that people eating diets rich in a nutrient have better outcomes. Trials then give that nutrient in isolated, often high-dose form — and the benefit frequently fails to appear. Beta-carotene supplements are the classic cautionary tale: diets rich in carotenoid-containing vegetables looked protective, but high-dose beta-carotene supplementation in smokers produced worse outcomes, not better. Food covers what you can’t measure. A diet gives you thousands of compounds — polyphenols, fibers, minor minerals — that no formula reproduces and no lab panel tracks. Supplement design can only include what’s already been identified as important.

A fourth, practical reason: it’s very hard to overdose on nutrients from food and quite easy to do so from capsules. Nobody develops vitamin A toxicity from carrots.

The Real Exceptions

Here’s where “just eat better” stops being adequate advice.

Vitamin D

The clearest case. Very few foods contain meaningful vitamin D — fatty fish, egg yolks, and fortified dairy, and that’s most of the list. Human vitamin D was never a dietary nutrient by design; it was a sunlight nutrient. Latitude, season, skin tone, sunscreen, indoor work, and age all reduce synthesis, and the higher latitudes get months per year where midday UVB is insufficient regardless of time outdoors.

The RDA is 600 IU/day for most adults and 800 IU/day after 70, with a tolerable upper limit of 4,000 IU/day. It’s fat-soluble, so take it with a meal containing fat. This is one of the few nutrients where testing is genuinely informative. See vitamin D explained.

Vitamin B12

There are no reliable plant sources. Claims about spirulina, nori, and fermented foods largely reflect B12 analogs that don’t function as the vitamin in humans — and may interfere with real B12. If you eat no animal products, fortified foods or a supplement isn’t optional.

RDA is 2.4 mcg/day; supplements typically run 25-1,000 mcg because oral absorption of large doses is inefficient. Adults over 50 are also advised toward fortified foods or supplements, since reduced stomach acid impairs release of B12 from food protein. Omega-3 EPA and DHA

Plant sources give you ALA (flax, chia, walnuts). Your body converts ALA to EPA and DHA, but inefficiently — conversion to EPA is often cited at under 10%, and to DHA far lower, with rates varying by sex and genetics. If you don’t eat fatty fish a couple of times a week, “eat more flax” doesn’t reliably close the EPA/DHA gap.

Iron — Conditionally

Not a universal gap, which is exactly why it needs care. Iron matters for menstruating women, pregnant women, endurance athletes, frequent blood donors, and people eating little or no heme iron. Everyone else generally doesn’t need it, and shouldn’t take it speculatively — excess iron accumulates, causes GI misery at best, and organ damage in people with hemochromatosis.

Test ferritin first. This is the single nutrient where “just try it and see” is the wrong instinct. Blood tests before supplementing covers which markers are worth measuring.

Iodine — Diet-Pattern Dependent

Iodized salt made this a solved problem for most people, but a few patterns reopen it: heavy use of non-iodized specialty salts, avoidance of dairy and seafood, and plant-based diets without a deliberate source. Pregnancy raises requirements meaningfully. Iodine has a narrow useful range — too much causes thyroid problems just as too little does — so this is a case for a modest, deliberate source rather than a large dose. See iodine explained.

Folate in Pregnancy — A Special Case

This one is unambiguous and worth separating from the rest: folic acid supplementation before and in early pregnancy is standard public-health guidance, at 400-800 mcg/day, precisely because food folate alone isn’t sufficiently reliable at the timing required. It’s the strongest example in nutrition of a supplement recommendation that food-first doesn’t override. Discuss it with your prenatal provider.

Where Food-First Holds Firm

The rest of the aisle is mostly duplication:

  • Vitamin C. A single bell pepper or orange covers the RDA. Deficiency in anyone eating any produce is rare.
  • Most B vitamins besides B12. Widespread in food and heavily fortified in grain products.
  • Magnesium and potassium. Common shortfalls in Western diets — but the fix is nuts, legumes, greens, and potatoes, which deliver far more per unit effort than a capsule. Magnesium supplements have a legitimate role at 200-400 mg/day for people who can’t close the gap, but food is the better lever.
  • Protein powder. A convenience product, not a nutritional necessity. Useful when hitting protein targets is logistically hard; not superior to food.
  • Antioxidant blends. The strongest argument against them is the trial record above.
  • “Whole food” multivitamins. Marketing that borrows food-first credibility for a product that’s still isolated nutrients. Covered in whole food vitamins myth.

A Decision Sequence

  1. Name the specific nutrient. “I feel tired” isn’t a nutrient. If you can’t name what you’re targeting, you’re shopping, not supplementing.
  2. Ask whether your actual diet supplies it. Not your ideal diet — the one you ate last week.
  3. Ask whether food realistically can. For vitamin D, B12 on a vegan diet, and EPA/DHA without fish, the honest answer is often no.
  4. Test where testing is informative. Ferritin, vitamin D, B12. Most other nutrients aren’t usefully measured by routine panels.
  5. Fill the gap at a sensible dose, then re-check. Not the highest dose on the shelf. Supplement upper limits has the ceilings that matter.

The pattern that emerges: a well-justified regimen for most people is short — often two or three items — and boring. That’s a feature.

Bottom Line

Food first is correct as a default and useless as a slogan. The exceptions are specific and knowable: vitamin D for nearly everyone at higher latitudes, B12 for vegans and many adults over 50, EPA/DHA for people who don’t eat fish, iron and iodine depending on diet pattern and life stage, and folic acid in pregnancy. Nearly everything else in the aisle is duplicating what a decent diet already provides — and the isolated-nutrient trial record is a good reason not to assume more is better.
