Most supplement shopping starts with a symptom and a search bar. You’re tired, so you read about iron, then B12, then adrenal support, and forty minutes later there are four bottles in a cart. None of them were chosen because you knew something was low. They were chosen because tiredness is nonspecific and the internet is happy to fill that vacuum.
There’s a cheaper, more useful first step: find out what’s actually going on. A handful of standard blood tests — most of them routine, most of them inexpensive, many already ordered at an annual physical — will answer the majority of the questions people try to answer by guessing.
This guide covers what to ask for, roughly what the numbers mean, and which tests are worth ignoring entirely.
Why Testing Beats Guessing
Three reasons, in order of importance.
Symptoms overlap almost completely. Fatigue, brain fog, poor sleep, hair shedding, low mood, and cold hands are the shared symptom set of low iron, low B12, an underactive thyroid, poor sleep, depression, and simply being a busy adult. You cannot tell these apart from the inside. A blood draw can tell several of them apart in a morning.
Supplementing blind can mask a problem. This is the one that matters most. High-dose folic acid can normalize the anemia caused by B12 deficiency while the neurological damage continues underneath. Taking iron because you feel tired, when the real issue is a thyroid problem or a source of blood loss that needs investigating, delays the actual answer. The supplement isn’t the danger — the delay is.
Some nutrients are genuinely risky to take blind. Iron accumulates and is hard to excrete; taking it without knowing your levels is not benign, particularly for men and post-menopausal women, who rarely need it. The Core Five
If you ask for nothing else, ask for these.
1. Complete Blood Count (CBC)
The cheapest, most routine panel there is, and a genuinely broad screen. It tells you whether you’re anemic, and — through MCV, the average red cell size — hints at why. Small cells point toward iron; large cells point toward B12 or folate. It also flags white cell and platelet abnormalities that have nothing to do with supplements but that you’d want to know about.
2. Ferritin (Iron Stores)
Ask for ferritin specifically, not just hemoglobin. This is the single most common gap in fatigue workups. Hemoglobin is the last thing to fall — you can burn through your iron stores entirely and still show a “normal” hemoglobin for a long stretch, while feeling progressively worse.
Reference ranges vary by lab, but a ferritin below roughly 30 ng/mL is widely treated as low or borderline, and many clinicians working with menstruating patients or endurance athletes want to see it higher than that. One important caveat: ferritin rises with inflammation and infection, so a normal-looking result during an illness can be misleading. If there’s any doubt, a full iron panel — serum iron, transferrin saturation, TIBC — sorts it out.
If ferritin is low, the next question is why, and that question belongs to a doctor. Unexplained iron deficiency in men and post-menopausal women, in particular, is something to investigate rather than supplement over.
3. Vitamin D — 25-Hydroxyvitamin D
The test you want is 25(OH)D, not 1,25-dihydroxyvitamin D (a different test, ordered for different reasons, that will mislead you here).
Common interpretation: below 20 ng/mL (50 nmol/L) is deficient, 20-30 ng/mL is insufficient, and 30-50 ng/mL is the range most guidance targets. There’s genuine expert disagreement about the ideal upper end, and chasing very high numbers has no demonstrated benefit — vitamin D is fat-soluble and does have an upper limit worth respecting.
This test is especially worth doing if you live at higher latitude, spend most daylight hours indoors, have darker skin, cover up for sun or cultural reasons, or are over 65. Those are the groups where the guesswork most often turns out to be wrong in both directions.
4. Vitamin B12
Worth testing if you’re vegetarian or vegan, over 50, taking metformin or long-term acid-reducing medication, or dealing with unexplained fatigue or tingling in the hands and feet.
The catch: serum B12 is a blunt instrument. The bottom of the reference range is set low, and people can have functional deficiency with results that look technically normal. If your result lands in the low-normal 200-400 pg/mL band and you have symptoms, ask about methylmalonic acid (MMA) or homocysteine — both rise when B12 isn’t doing its job at the cellular level, and they catch what the serum test misses. 5. TSH (Thyroid)
Not a nutrient test, but it belongs on this list because thyroid dysfunction is the most common medical explanation for the exact symptom cluster that sends people to the supplement aisle. It’s cheap, routine, and rules out something important. If TSH is abnormal, that’s a medical conversation — not a supplement one, and emphatically not an iodine-buying one, since taking iodine into an undiagnosed thyroid problem can make things worse.
Worth Adding, Depending on You
- Folate — usually alongside B12, particularly if MCV is high or you’re pregnant or planning to be.
- HbA1c and a lipid panel — standard metabolic screening; useful context before buying anything marketed for blood sugar or cholesterol.
- Magnesium — with a large caveat. Standard serum magnesium is a poor reflection of body stores, since the body defends blood levels tightly while tissue stores drop. RBC magnesium is somewhat more informative but isn’t standard everywhere and is still imperfect. This is an area where a normal result shouldn’t fully reassure you and where dietary intake is often the more useful question. Our magnesium guide covers what intake actually looks like in practice.
- Omega-3 index — a specialty test, not usually necessary. Fine if you’re curious; not a prerequisite.
Tests to Skip
Hair mineral analysis. Repeatedly shown to give inconsistent results — the same hair sample sent to different labs can come back with different findings and contradictory recommendations. Not a valid basis for supplementation.
Most direct-to-consumer “micronutrient panels.” The expensive ones testing dozens of nutrients at once look thorough and mostly aren’t. Many individual assays are poorly validated, reference ranges are shaky, and testing 40 things guarantees a few false alarms by chance alone — each of which comes attached to a product recommendation. Note who’s selling the test and whether they also sell the fix.
Food sensitivity IgG panels. Not a nutrient test at all, but they travel in the same circles. Mainstream allergy and immunology bodies do not support them for diagnosing food intolerance.
“Intracellular” and specialty antioxidant panels. Interesting in research settings, not validated for individual clinical decisions.
How to Actually Ask
Bring it as a question, not a printout of demands. Something like: “I’ve been tired for a few months and I’d rather find out what’s going on than start buying supplements. Could we check a CBC, ferritin, vitamin D, B12, and TSH?”
That framing works because it’s the same reasoning your doctor uses. Most of those tests are routine, defensible, and inexpensive. If yours declines one, ask what they’d order instead — there may be a good reason.
Two practical notes: fasting is generally required for lipids and glucose, so book a morning slot, and pause biotin supplements for a few days first if you take them. High-dose biotin interferes with several common lab assays — including thyroid tests — and can produce dramatically wrong results. Tell the lab what you’re taking.
Reading Results Without Panicking
A result just outside the reference range is not automatically a problem. Ranges are statistical constructs built from a reference population, and a value can sit slightly outside one while being entirely normal for you. Context — your symptoms, your history, your other results — is what turns a number into a decision. That’s the clinician’s job, not a search engine’s.
If something is genuinely low, treat the correction as a defined project: take a dose appropriate to the gap, for a defined period, then retest at 8-12 weeks. Most nutrient levels take that long to move meaningfully, and retesting at three weeks mostly tells you that you’re impatient. Ferritin in particular can take months to rebuild.
Then reassess. Some deficiencies are one-time corrections with a fixable cause. Others — B12 in a vegan diet, vitamin D at high latitude in winter — are ongoing situations that need a maintenance dose. Knowing which is which is the entire value of having tested in the first place.
Bottom Line
Five ordinary tests — CBC, ferritin, 25(OH)D, B12, TSH — resolve most of the guesswork that drives supplement buying, and they cost less than a few months of bottles you didn’t need. Test first, correct what’s actually low, retest at 8-12 weeks, and let a clinician read the numbers in context.
