Biotin Explained: What Vitamin B7 Does, How Much You Need, and the Lab Test Problem Nobody Mentions

The vitamin in every hair gummy, and the one that fools your blood work.

Biotin has a strange reputation. It is one of the least likely vitamins for anyone to be deficient in, yet it appears in enormous doses in some of the best-selling supplements on the market. Most people who take it are chasing thicker hair or stronger nails. Few of them know that the dose in their gummy can throw off a thyroid panel or, in rare cases, mask a heart attack on a blood test.

This guide covers what biotin actually does, how much you need, who genuinely runs short, what the hair and nail evidence looks like, and the lab interference issue that deserves far more attention than it gets.

What biotin does

Biotin is vitamin B7, sometimes called vitamin H in older literature. Like the other B vitamins, it’s water-soluble, meaning your body doesn’t store much of it and excretes the excess in urine. We cover the general implications of that in our water-soluble vs. fat-soluble guide.

Its job is to serve as a cofactor for a small family of enzymes called carboxylases. These enzymes sit at key points in metabolism:

  • Fatty acid synthesis, building the lipids that go into cell membranes and, yes, hair and skin.
  • Gluconeogenesis, the process of making glucose from non-carbohydrate sources when you haven’t eaten.
  • Amino acid breakdown, particularly of leucine and a few others.

Biotin also plays a less-understood role in regulating gene expression. Because these enzymes are involved in making the building blocks of skin, hair, and nails, deficiency shows up in those tissues, which is the grain of truth behind the marketing.

How much you need

There isn’t enough data to set a formal recommended dietary allowance for biotin, so nutrition authorities use an “adequate intake” instead. For adults it’s 30 micrograms per day, rising to 35 mcg during breastfeeding. Children need less, scaling with age.

Thirty micrograms is a small number. A single cooked egg provides roughly a third of it. Add some nuts, seeds, salmon, sweet potato, or a serving of liver, and you’re comfortably over the line. Gut bacteria also synthesize some biotin, though how much of that gets absorbed is debated.

No tolerable upper intake level has been set, because no toxicity has been documented even at very high doses. That’s often quoted as if it means megadoses are a good idea. It doesn’t. It means nobody has been harmed by the biotin itself. The lab test issue below is a different kind of harm entirely.

Who actually becomes deficient

Genuine biotin deficiency is rare in people eating a varied diet. When it does happen, it’s usually one of these situations:

  • Biotinidase deficiency, a genetic condition screened for at birth in most countries, in which the body can’t recycle biotin. These individuals need lifelong supplementation under medical care.
  • Long-term anticonvulsant use. Several seizure medications reduce biotin absorption or increase its breakdown. People on these drugs for years can drift into marginal status.
  • Chronic heavy alcohol use, which impairs absorption of most B vitamins.
  • Regular consumption of raw egg whites. Raw egg white contains avidin, a protein that binds biotin tightly and prevents absorption. Cooking denatures avidin, so cooked eggs are a biotin source, not a problem. This is a real issue mainly for people drinking raw egg protein shakes daily.
  • Pregnancy. Some research suggests marginal biotin status is more common in pregnancy than assumed, though the clinical significance is unclear. Standard prenatal vitamins cover it.
  • Long-term intravenous nutrition without biotin added, which is now rare.

Deficiency signs include thinning hair, a scaly red rash around the eyes, nose, and mouth, brittle nails, and in more severe cases, neurological symptoms like tingling, depression, and lethargy. If you have several of these together, that’s a reason to talk to a provider, not to self-diagnose from a list.

The hair and nail question

This is why most people take biotin, so let’s be direct about the evidence. We wrote a full post on biotin and hair growth, but the short version:

In people who are deficient, biotin supplementation reverses hair loss and nail brittleness. That’s well documented and not in dispute.

In people who aren’t deficient, which is nearly everyone buying hair gummies, the evidence is weak. The studies that get cited are small, often lack a placebo group, and frequently involve people who had an underlying deficiency or a condition affecting biotin status. There is no good controlled trial showing biotin thickens hair in healthy adults with normal intake.

For nails, the picture is slightly better. A few small older studies found that around 2.5 mg per day for several months improved nail thickness and reduced splitting in people with brittle nails. Those studies are dated and small, but the finding has been consistent enough that biotin for brittle nails is a reasonable low-risk trial if nothing else has helped.

Typical hair-and-nail products supply 2,500 to 10,000 mcg (2.5 to 10 mg), which is 80 to 300 times the adequate intake. There is no evidence that 10 mg does anything 2.5 mg doesn’t, and the higher doses are the ones most likely to cause the problem below.

The lab test problem

This is the part of the biotin story that matters most, and it’s the part almost no supplement label mentions.

Many common blood tests use a technology called immunoassay, and a large share of those assays use biotin as part of their chemistry. When someone has high levels of biotin circulating in their blood from supplements, it can interfere with the test and produce results that are falsely high or falsely low, depending on the assay design.

Tests known to be affected include:

  • Thyroid tests (TSH, free T4, free T3). Biotin interference can make a normal thyroid look overactive, leading to unnecessary workups or even inappropriate treatment.
  • Troponin, the marker used in emergency rooms to detect a heart attack. Biotin can cause a falsely low troponin, which is the dangerous direction. Regulators have issued warnings about this specifically.
  • Vitamin D, B12, folate, and various hormone tests, including reproductive hormones and cortisol.
  • Some prostate, pregnancy, and cardiac markers.

The interference is dose-dependent. At the 30 mcg in a standard multivitamin or B-complex, it’s not a concern. At 5 to 10 mg, which is what many hair supplements contain, it is. Some case reports involve people taking even higher doses for other reasons.

What to do about it: stop biotin supplements at least 48 to 72 hours before scheduled blood work, and always tell your provider and the lab that you take it. If you end up in an emergency setting, mention it there too. This is one of the very few supplement facts we’d put in the “everyone should know this” category.

Forms, dosing, and timing

Supplements use D-biotin, the biologically active form. There’s no meaningful difference between brands beyond dose and third-party testing.

  • For general nutrition: the 30 to 300 mcg in a multivitamin or B-complex is plenty. Most people don’t need a standalone product at all.
  • For brittle nails: 2.5 mg per day for at least three to six months, with the lab test precaution above.
  • For diagnosed deficiency or medication-related depletion: dosing should be set by a provider.

Timing doesn’t matter much. Biotin absorbs fine with or without food, and because it’s water-soluble, there’s no need to pair it with fat.

Safety and interactions

Biotin itself has an excellent safety record. Excess is excreted, and there are no confirmed cases of toxicity. Some people report breakouts on high doses; the evidence for that is anecdotal but common enough to mention.

Things worth knowing:

  • Anticonvulsants lower biotin status, as noted above. Anyone on long-term seizure medication may want to discuss testing.
  • Antibiotics taken long-term can reduce the gut bacteria that produce biotin, though the practical impact is small.
  • Alpha-lipoic acid shares a transporter with biotin, and very high doses might compete for absorption. This is theoretical and unlikely to matter at normal doses.
  • Pregnancy and nursing: the amounts in a prenatal vitamin are appropriate and safe. Megadoses for hair haven’t been studied in pregnancy, so skip them unless a provider recommends otherwise.

Bottom line

Biotin is an essential nutrient that a normal diet supplies without effort. Supplementing it fixes hair and nail problems only when a deficiency was the cause, which is uncommon. If you want to try it for brittle nails, 2.5 mg daily for a few months is a reasonable and low-risk experiment. Whatever dose you take, stop it for two to three days before any blood test and tell your provider, because high-dose biotin can quietly corrupt the results.
