If your feed looks anything like ours did this summer, you’ve seen the ritual: half a cup of orange juice, a splash of coconut water, a quarter teaspoon of cream of tartar, a pinch of sea salt, stirred in a pretty glass and sipped mid-afternoon. The captions promise it will “lower cortisol,” “heal your adrenals,” fix the 3 p.m. crash, the belly fat, the 3 a.m. wake-ups, the puffy face.
The recipe is harmless for most people and honestly kind of pleasant. The claims wrapped around it are another matter. Let’s separate the drink from the diagnosis it’s riding on.
The Premise Problem: “Adrenal Fatigue” Isn’t a Thing
The cortisol cocktail (also sold as the “adrenal cocktail”) only makes sense inside a specific story: that chronic everyday stress overworks your adrenal glands until they’re too tired to make hormones properly, leaving you wired-but-tired — and that targeted nutrients can nurse them back.

Chronic stress is real, and cortisol rhythms genuinely matter. But cortisol regulation happens in the brain — the hypothalamic-pituitary-adrenal axis — and responds to sleep, light exposure, workload, caffeine timing, and psychological stress management. Not to cream of tartar.
What’s Actually in the Glass
Run the standard recipe through a nutrition database and the mystique evaporates fast. A typical serving — ½ cup orange juice, ½ cup coconut water, ¼ tsp cream of tartar, ⅛-¼ tsp salt — delivers approximately:
- Sodium: 400-600 mg (mostly from the salt)
- Potassium: 500-700 mg (cream of tartar is potassium bitartrate at roughly 250 mg potassium per ¼ tsp, plus generous contributions from the OJ and coconut water)
- Vitamin C: 50-80 mg (from the juice)
- Sugar: 12-18 g, water, and trace magnesium
In other words: a homemade electrolyte drink with a vitamin C garnish. There’s nothing wrong with that — it’s compositionally similar to an oral rehydration mix or a sports drink with better ingredients. But nothing in it has a demonstrated effect on cortisol output, cortisol rhythm, or adrenal gland function in humans. Vitamin C is involved in adrenal hormone synthesis, which is the kernel of truth the marketing grew from — but being a cofactor is not the same as being a lever. Your adrenals are not vitamin-C-starved at 50 mg increments, and no trial shows this drink moves any stress hormone measurably.
And to be clear about the evidence landscape: no controlled trial has ever tested the cortisol cocktail against placebo for stress, energy, sleep, cortisol levels, or anything else. Every claim you’ve seen is testimonial.
So Why Do People Swear It Works?
Because for a decent slice of its audience, it’s fixing something real — just not the thing on the label:
- Dehydration. Many people run mildly under-hydrated, especially in summer heat. Fluid plus sodium plus potassium genuinely improves how you feel.
- Under-fueling. The drink’s biggest fan base overlaps heavily with people skipping meals or on aggressive diets. Fifteen grams of quick sugar at the 3 p.m. slump will perk you up.
- Caffeine displacement. The ritual often replaces a third coffee. Less afternoon caffeine means better sleep, which means less of the wired-but-tired feeling the drink was supposed to cure.
- Ritual and expectation. A deliberate pause with a pretty drink is a micro-break, and expectation effects on subjective energy are strong — the same placebo dynamics that flatter most wellness trends.

If You Like the Drink, Keep the Drink — Fix the Label
There are contexts where a sodium-potassium drink with some quick carbohydrate makes straightforward sense: after a long sweaty workout, during a heat wave, on a day you’ve been outside for hours. Who Should Actually Be Careful
“It’s just juice” isn’t quite the whole safety story, because the recipe is deliberately salt- and potassium-loaded, and some versions on social media go much heavier:
- High blood pressure or heart conditions: an extra 400-600 mg of sodium per glass, daily, is meaningful against typical intake guidance — most adults already exceed recommended sodium limits.
- Kidney conditions, or medications that raise potassium — ACE inhibitors, ARBs, potassium-sparing diuretics: a 500-700 mg potassium bolus per drink is exactly the kind of “hidden” potassium source clinicians ask about. Check first.
- Blood-sugar management: it’s a sugared drink; treat it like one.
- Pregnancy and nursing: the ingredients are ordinary foods and generally fine — but the impulse behind the drink (“my hormones are off”) deserves an actual clinician conversation instead.
If Afternoon Crashes Are Your Real Problem
The evidence-supported checklist is boring and effective: consistent sleep and wake times, morning light, lunch that contains actual protein, caffeine cut off by early afternoon, and honest stress outlets.
