Metformin and Vitamin B12: What Long-Term Users Should Know

A decades-old drug, a well-documented side effect, and a simple habit that deals with it.

Metformin is one of the most widely prescribed medications in the world. It’s inexpensive, has decades of safety data, and is the usual first-line drug for type 2 diabetes. It’s also used for other conditions, including polycystic ovary syndrome. Millions of people take it every day for years at a time.

It also has one of the better-documented drug-nutrient interactions in medicine: long-term metformin use is linked to lower vitamin B12 levels. That isn’t a reason to fear the drug. It’s a reason to add one routine blood test to your care. Here’s what the research shows and what to do about it.

What the research shows

Evidence that metformin lowers B12 comes from several kinds of studies, and they point in the same direction:

  • Randomized trials comparing metformin with placebo have found lower B12 levels in the metformin groups over follow-up periods of several years.
  • Observational studies of large patient populations consistently find a higher rate of low or borderline B12 among long-term metformin users than among non-users.
  • Dose and duration matter. The link is generally stronger at higher daily doses (studies often point to around 1,500 mg/day and above) and after several years of use rather than months.

Estimates of how many long-term users develop low B12 vary a lot between studies, depending on the population, the test used, and the cutoff chosen. What’s consistent is that the effect is real, builds slowly, and is common enough that routine monitoring is worthwhile.

Why it happens

B12 absorption is a multi-step process. After stomach acid releases B12 from food, it binds a protein called intrinsic factor, and the combined complex is absorbed in the last section of the small intestine. That final absorption step depends on calcium.

The leading hypothesis is that metformin interferes with that calcium-dependent step. Other proposed contributors include changes in gut bacteria and in how quickly food moves through the intestine. The exact mechanism isn’t fully settled. A small early study suggested extra calcium might partly reverse the effect, but that finding is preliminary and isn’t a reason to start taking calcium for this purpose.

Why it matters, and why it’s easy to miss

Vitamin B12 is needed to make red blood cells and to maintain nerves. When levels fall low enough for long enough, possible effects include:

  • A type of anemia with enlarged red blood cells, which can cause fatigue and shortness of breath
  • Tingling, numbness, or burning in the hands and feet
  • Balance problems, memory changes, or low mood in more advanced cases

Here’s the catch for people with diabetes: tingling and numbness in the feet are also classic signs of nerve damage from diabetes itself. When a long-term metformin user reports those symptoms, it’s easy to assume diabetes is the only cause. Checking B12 is how you tell a contributing, correctable factor from the underlying condition.

This is also why B12 status can drift for years without anyone noticing. The decline is gradual, and early symptoms are vague.

What guidelines recommend

Major diabetes care guidelines recommend periodic measurement of B12 in people taking metformin long term, especially if they have anemia or nerve symptoms. There’s no single mandated schedule. Many clinicians check at baseline and then every year or two, or sooner if symptoms appear.

If you’ve been on metformin for a few years and can’t remember ever having your B12 checked, that’s a perfectly reasonable thing to bring up at your next appointment.

Which tests to ask for

Serum B12 is the standard first test. It’s cheap and widely available, but it has a gray zone: results in the low-normal range don’t always reflect what’s happening in your tissues.

If your result is borderline, a clinician may add methylmalonic acid (MMA) and sometimes homocysteine. These rise when your cells don’t have enough B12 to work properly, so they’re better at catching early, functional deficiency. How supplementation works

If testing shows low or borderline B12, the usual approach is to supplement, not to change your diabetes medication.

  • Oral B12 works for most people. Intrinsic-factor absorption is limited, but about 1% of a large oral dose is also absorbed by passive diffusion, which doesn’t depend on that pathway. That’s why oral doses of 250-1,000 mcg a day can restore levels even when normal absorption is impaired.
  • Injections are an option for severe deficiency, significant nerve symptoms, or when oral supplements don’t raise levels enough. That decision belongs to your clinician.
  • Form matters less than marketing suggests. Cyanocobalamin is the most studied and most stable form. Methylcobalamin also works. Our vitamin B12 page compares them.
  • Timing is flexible. B12 can be taken with or without food, and at any time of day.

B12 has no established upper limit because excess is poorly absorbed and readily excreted, and even high oral doses are well tolerated in healthy people. Still, supplementing blindly has two drawbacks. It can mask the reason you were low, and very high blood levels can make later test results harder to interpret. Test, supplement based on the result, and retest.

Other factors that stack the odds

The risk from metformin adds up with other common factors that lower B12:

  • Acid-reducing drugs (proton pump inhibitors and H2 blockers), which reduce the stomach acid needed to release B12 from food
  • Age over about 50, when stomach acid production often declines
  • Vegan or vegetarian diets without a reliable B12 source
  • Previous stomach or intestinal surgery, including some weight-loss procedures

Someone over 60 on both metformin and a daily acid reducer has several of these factors at once and is a strong candidate for regular testing.

One more note on folate. High-dose folic acid supplements can correct the anemia caused by low B12 while the nerve effects continue. If you take a B-complex or high-dose folic acid, B12 testing becomes more important, not less.

Safety notes

  • Don’t stop, skip, or cut back on metformin to protect your B12 levels. It’s an effective medication, and the B12 issue is easy to manage alongside it.
  • Be careful with “blood sugar support” supplements. Several herbal products marketed for glucose control can add to the effect of metformin and other diabetes drugs, which raises the risk of low blood sugar. Talk to your prescriber before adding any of them.
  • Kidney disease: If you have significant kidney disease, ask your clinician which B12 form and dose is right for you.
  • Pregnancy and PCOS: If you take metformin for PCOS or during pregnancy, your prescriber should guide B12 testing and any supplementation, alongside your prenatal plan.

Bottom line

Long-term metformin use can gradually lower vitamin B12, especially at higher doses and after several years. The response is simple: ask for periodic B12 testing (with MMA if the result is borderline), and supplement with oral B12 if your levels are low. Keep taking your metformin, and treat new tingling or numbness as a reason to test rather than assuming it’s only diabetes.
