How Much Vitamin B6 Is Too Much? What the Research Actually Shows

The water-soluble vitamin that breaks the 'you just pee it out' rule.

There’s a comfortable piece of folk wisdom about water-soluble vitamins: your body takes what it needs and excretes the rest, so the worst outcome of overdoing it is expensive urine. We’ve written about where that idea holds and where it doesn’t, and vitamin B6 is the headline exception — the one water-soluble vitamin with a well-characterized toxicity pattern and a formal upper limit that supplement products routinely approach or exceed.

This isn’t a reason to fear B6. It’s a reason to know your number.

What B6 Does and How Much You Need

Vitamin B6 is a family of related compounds — pyridoxine, pyridoxal, pyridoxamine and their phosphorylated forms — that converge on the active coenzyme pyridoxal 5′-phosphate (P5P). P5P is a workhorse cofactor for well over a hundred enzymatic reactions, most of them involving amino acid metabolism, plus steps in the production of several neurotransmitters, in hemoglobin synthesis, and in methylation-related pathways.

Food covers this readily for most people. Chickpeas, tuna, salmon, poultry, potatoes, bananas, and fortified cereals are all meaningful sources. Frank deficiency is uncommon in the general population; it shows up more often alongside alcohol use disorder, certain kidney conditions, malabsorption, and with a handful of medications that deplete B6 — isoniazid being the classic example, which is why B6 is typically prescribed alongside it.

The Upper Limit and Where It Came From

The US tolerable upper intake level (UL) for adults is 100 mg/day from supplemental sources. That figure was set specifically because of the neurological effect described below, with a safety margin applied.

Worth knowing: this is one of the nutrients where authorities disagree noticeably. Several European bodies have set considerably lower limits — in the region of 12–25 mg/day — after reviewing case reports occurring below the US threshold. When two competent agencies land an order of magnitude apart, the honest read is that the true no-effect threshold isn’t precisely known, and that a conservative posture is defensible. The Actual Finding: Sensory Neuropathy

The consistent adverse effect associated with excessive B6 is a sensory peripheral neuropathy — the nerves that carry sensation, particularly from the hands and feet.

What the literature describes:

  • The earliest historical descriptions involved very large doses — grams per day, taken for months, in an era when megadose B6 was popular for premenstrual symptoms and carpal tunnel discomfort. In those reports the effect was pronounced.
  • Subsequent case reports and case series have described symptoms at substantially lower intakes, in some instances in the tens of milligrams per day, when taken over long periods. These are reports, not controlled trials, and confounding is hard to exclude — but there are enough of them to take seriously.
  • The relationship appears to be a function of both dose and duration. Short courses at moderate doses under clinical supervision have a different risk profile from years of daily high-dose self-supplementation.
  • Symptoms typically improve after stopping, sometimes over months. Recovery is not always complete in the more severe reported cases.

Reported symptoms include tingling, numbness, burning or electric sensations in the hands and feet, reduced sense of vibration or position, and unsteadiness when walking — particularly in the dark, when visual compensation is removed.

An important caveat: these are also symptoms of many other things, several of them common and unrelated to supplements. Peripheral nerve symptoms warrant an evaluation, not a self-diagnosis. If you notice them, tell a clinician what you’re taking and let them work it out. Bring the bottles.

Why Ordinary Stacks Get There

Nobody sets out to take 150 mg of B6. It accumulates because B6 appears in a lot of products at doses far above the 1.3 mg you need, and no single label looks unreasonable in isolation.

A realistic audit:

  • B-complex supplement: commonly 25–100 mg of B6, occasionally more. This is usually the dominant contributor.
  • Multivitamin: often 2–25 mg.
  • Energy drinks: frequently a few mg to over 10 mg per can, and multiple cans a day isn’t unusual.
  • Pre-workout formulas: commonly include B6 in the same range.
  • “Nerve support,” PMS, mood, and sleep formulas: these often carry the highest single doses, sometimes 50–100 mg, which is an uncomfortable irony given the adverse effect in question.
  • Fortified foods: modest, but not zero.

Is P5P Safer Than Pyridoxine HCl?

You’ll see this claim often. The reasoning is that pyridoxine HCl must be converted to the active P5P form, and that supplying the active form directly avoids some hypothesized mechanism by which unconverted pyridoxine interferes with nerve tissue.

It’s a mechanistically plausible hypothesis. It is not settled. There are case reports involving P5P products too, and controlled comparative safety data in humans is limited. The honest position is: P5P may be preferable, but “may” is doing real work in that sentence, and it should not be treated as a licence to take a higher dose. Interactions and Special Situations

Levodopa without carbidopa. B6 accelerates peripheral metabolism of levodopa, reducing how much reaches the brain. Most modern formulations combine levodopa with carbidopa, which largely blunts this — but anyone on Parkinson’s medication should confirm B6 intake with their neurologist rather than assuming.

Anticonvulsants. Interactions in both directions have been described with some agents, including phenytoin and phenobarbital. Worth a pharmacist conversation.

Isoniazid, cycloserine, hydralazine, penicillamine. These deplete B6, and supplementation is often deliberately prescribed alongside them — at a dose your prescriber sets.

Pregnancy. B6 is used clinically for pregnancy-related nausea, typically in the 10–25 mg range and often in a combination product, under medical direction. That’s a supervised use with a defined duration — not a reason to self-select a high-dose product. See supplement safety during pregnancy.

Kidney impairment. Clearance of water-soluble vitamins changes with reduced kidney function; doses that are fine for others may not be. Individualized advice applies.

A Sensible Approach

  • If you eat a reasonably varied diet, you probably need no supplemental B6 at all.
  • If you take a B-complex, check whether you need one with 100 mg of B6 or whether a lower-dose version does the job. Many do.
  • Keep long-term supplemental intake modest — staying near or below the low double digits in milligrams for open-ended daily use is a defensible conservative target given the disagreement between authorities.
  • Reserve higher doses for situations a clinician has specifically directed, with a defined duration.
  • Re-audit whenever you add a product, since the risk lives in the total, not the item.

Bottom Line

Vitamin B6 is essential, easy to get from food, and safe at the intakes most people actually need. It also has a genuine dose- and duration-dependent adverse effect on sensory nerves — the exception that breaks the “water-soluble vitamins are harmless” rule. The US upper limit is 100 mg/day; some European authorities set theirs far lower; and ordinary supplement stacks can reach those numbers without anyone noticing. Add up your total, keep long-term daily doses modest unless a clinician has said otherwise, and treat new numbness or tingling as a reason to see someone rather than to search online.
