Gotu kola (Centella asiatica) has one of the longest continuous traditional-use records of any herb in the supplement aisle — Ayurvedic and traditional Chinese medicine both feature it, and it shows up as a food in parts of Southeast Asia. Long use is a reasonable prior for basic tolerability. It is not evidence of effect, and the gap between what gotu kola is sold for and what has actually been tested in people is wide.
Here’s what the research supports, at what doses, and where to stop believing the label.
What it actually is
Gotu kola is a low-growing wetland plant in the parsley family. Its active constituents are pentacyclic triterpenes — principally asiaticoside, madecassoside, asiatic acid, and madecassic acid. Standardized products concentrate these, often as a “total triterpenic fraction of Centella asiatica” (TTFCA) or “titrated extract” (TECA), typically standardized to somewhere between 40% and 95% triterpenes.
This distinction matters more than usual. A capsule of dried gotu kola leaf powder and a standardized triterpene extract are chemically different products at wildly different potencies, and virtually all the positive clinical work used the standardized fraction. If a label says “gotu kola 1,000 mg” with no triterpene percentage, you cannot map it onto the trial doses. One naming confusion worth clearing up: gotu kola is sometimes sold as “brahmi,” a name also used for bacopa monnieri. They’re unrelated plants with different evidence bases. If you’re buying for cognition, bacopa has the better data — and it’s covered in our bacopa research brief.
The strongest evidence: venous circulation
The area with genuine repeated human trials is chronic venous insufficiency symptoms — the leg heaviness, aching, ankle swelling, and end-of-day discomfort that comes from veins struggling to return blood upward.
Multiple small randomized trials, mostly using 60-180 mg/day of standardized triterpene fraction over 4 to 12 weeks, report improvement in subjective symptoms and in some objective measures like ankle circumference and capillary filtration rate. The proposed mechanism is stabilization of connective tissue in the vessel wall — gotu kola triterpenes appear to modulate collagen synthesis in vascular tissue.
The honest caveats: the trials are small, several are decades old, and a meaningful share came from investigators connected to the manufacturers of the standardized extracts. That doesn’t invalidate the findings, but it’s the same replication problem that affects most single-source botanical extracts, and it belongs in the assessment.
For the same complaint, horse chestnut and diosmin-hesperidin have comparable or somewhat better-supported evidence. Gotu kola is a reasonable member of that group, not the standout its marketing implies.
Skin and wound healing: mostly topical
Gotu kola is a fixture in skincare, and there’s a real basis for it — asiaticoside and madecassoside promote collagen synthesis and have been studied in wound healing and scar management, generally in topical preparations around 1% asiaticoside.
The leap that marketing makes is from topical to oral. Applying a triterpene extract to healing tissue and swallowing a capsule are not the same intervention, and oral trials for skin outcomes — wrinkles, elasticity, hydration — are sparse and small. If skin is your goal, the topical evidence is stronger than the oral evidence, and oral collagen peptides have more human data behind them for skin elasticity endpoints.
Cognition, mood, and stress: preliminary
This is where most of the online enthusiasm lives and where the evidence is thinnest.
There are small trials suggesting effects on anxiety measures, acoustic startle response, mood, and some cognitive endpoints in older adults. They tend to be short, small (often under 60 participants), varied in dose and preparation, and largely unreplicated. Some used crude extracts at 500-750 mg twice daily rather than standardized fractions, which makes comparison across studies difficult.
Read that as: plausible, worth further study, not established. Anyone selling gotu kola as a memory or anxiety solution is well ahead of the data. It’s also not an adaptogen in the technical sense our adaptogens guide describes, despite frequently being marketed inside that category.
Dosing and timing
- Standardized triterpene fraction: 60-180 mg/day, usually split into 2-3 doses. This is the range used in the venous circulation trials.
- Crude/dried extract: roughly 500-1,000 mg twice daily, which is what most general supplements provide. Weaker evidence base.
- With food: Generally better tolerated; some people report nausea otherwise.
- Topical: Around 1% asiaticoside preparations for skin use.
- Timeline: Circulation trials ran 4-12 weeks. Don’t judge it at two weeks.
- Cycling: Traditional and European herbal practice suggests roughly 6 weeks on, 2 weeks off. Given the liver signal below, that caution is worth keeping.
Safety and interactions
Gotu kola is generally well tolerated at studied doses, but it has a longer caution list than most gentle-sounding herbs:
- Liver. Rare case reports of drug-induced liver injury exist, mostly with prolonged use. Avoid it if you have liver disease, hepatitis, or are taking anything hepatotoxic — and don’t stack it with other liver-stressing supplements. The cycling convention exists for this reason.
- Sedatives. May add to the effect of sedative medication, sleep aids, and alcohol. Use caution.
- Surgery. Stop two weeks beforehand, both for the sedation interaction and general prudence.
- Pregnancy and nursing. Avoid. Safety hasn’t been established, and traditional sources flag it.
- Blood sugar and cholesterol medication. Possible additive effects reported; monitor if applicable.
- Skin reactions. Contact dermatitis occurs with topical use in a minority of people. Patch test.
There’s no established tolerable upper intake level, and doses well above the studied range have no supporting evidence — the general logic in our upper limits guide applies.
What to look for on the label
Because the potency gap between preparations is so large, three things decide whether a gotu kola product is worth buying:
- A stated triterpene percentage. “Standardized to 10% (or 40%, or 95%) total triterpenes” lets you calculate the actual active dose. A milligram figure alone does not. If a 1,000 mg capsule is standardized to 10% triterpenes, you’re getting 100 mg of the fraction — squarely in the studied range. Unstandardized leaf powder at the same milligram count may deliver a small fraction of that.
- Aerial parts, identified by botanical name. Centella asiatica should appear on the label. Because “brahmi” is applied to two different plants, common names alone are not reliable identification, and adulteration and substitution are documented problems in the botanical supply chain.
- No proprietary blend. Gotu kola is frequently buried in “circulation” or “beauty” formulas at undisclosed amounts. If the individual dose isn’t listed, assume it’s below what the trials used — that’s usually the reason it’s hidden.
Price should be modest. Gotu kola is not a scarce botanical, and a premium usually reflects packaging or third-party verification rather than a stronger extract.
Bottom line
Gotu kola has one reasonably supported use — reducing leg heaviness and swelling from poor venous return, at 60-180 mg/day of a standardized triterpene fraction over 4-12 weeks. Its skin evidence is real but mostly topical, and its cognition and anxiety claims are preliminary at best. If you use it, buy a product that states its triterpene standardization, cycle it rather than taking it indefinitely, and skip it entirely if you’re pregnant, nursing, or have any liver concern.
