Cocoa flavanols are in an unusual position for a plant compound. Most polyphenols get sold on the strength of lab studies and a handful of tiny human trials. Cocoa flavanols have dozens of controlled trials on blood-vessel function, a European health-claim authorization, and — rarely for a supplement — a large, multi-year randomized trial with a cardiovascular outcome.
That doesn’t make them a miracle. It makes them a good case study in what “decent evidence” looks like for a supplement: effects that are consistent, measurable, and small. Here’s where the research stands, what was actually dosed, and why the chocolate aisle is the wrong place to shop for this.
What Cocoa Flavanols Are

In cocoa, the key members are epicatechin, catechin, and chains of them called procyanidins. Epicatechin gets most of the mechanistic credit: it appears to increase nitric oxide availability in the lining of blood vessels, which relaxes them and improves blood flow.
Raw cocoa beans are rich in these compounds. The problem is that almost everything done to cocoa on the way to becoming food destroys them:
- Fermentation and roasting reduce flavanol content substantially.
- Alkalization (“Dutch processing”), used to mellow flavor and darken color, can wipe out 60–90% of what’s left.
- Dilution with sugar, milk, and fat in finished chocolate lowers the concentration further.
A typical dark chocolate bar might deliver anywhere from a few dozen to a couple hundred milligrams of flavanols — and the label almost never tells you, because cocoa percentage is a poor proxy for flavanol content. Supplements and specially processed cocoa powders are standardized to stated flavanol amounts, which is the only way to know what you’re taking.
What the Trials Found
Blood-vessel function: the most consistent result
The best-replicated finding is on flow-mediated dilation (FMD), a test of how well an artery widens in response to increased blood flow. Across many small crossover trials, cocoa flavanol doses in the range of roughly 200–900 mg produced measurable improvements in FMD within hours, and sustained improvements with daily intake over weeks. This is the basis for the European Food Safety Authority’s authorized claim that 200 mg of cocoa flavanols daily “helps maintain the elasticity of blood vessels, which contributes to normal blood flow.” Note the careful wording — it’s about maintaining normal function, not fixing a problem.
Blood pressure: small but real
Pooled analyses of randomized trials generally show reductions on the order of 1–4 mmHg in systolic blood pressure with flavanol-rich cocoa versus low-flavanol control. That’s comparable to what we’ve discussed for beetroot nitrates — meaningful at a population level, unlikely to be something you notice personally, and not a substitute for anything a clinician has prescribed. Effects tend to be larger in people with higher starting blood pressure and smaller in people already in a healthy range.
Cardiovascular events: one big trial, a cautious signal
The piece of evidence that separates cocoa from most polyphenols is a large randomized, placebo-controlled trial that gave older adults 500 mg/day of cocoa flavanols (about 80 mg epicatechin) for several years and tracked actual cardiovascular events. The headline result missed statistical significance for the primary combined outcome. But the trial found fewer cardiovascular deaths in the flavanol group, and a secondary analysis restricted to people who took their capsules consistently showed a stronger effect.
How to read that honestly: it’s a promising signal, not a confirmed benefit. Secondary and “per-protocol” analyses are more vulnerable to bias than the primary analysis, and the trial was not designed to prove a mortality effect. Researchers have generally called for a confirmatory trial rather than declaring victory. We’d put it the same way.
Cognition: mixed, leaning disappointing
Short trials — days to a few months — have reported improvements in tasks like attention, processing speed, and memory with high-flavanol cocoa, sometimes alongside measured increases in brain blood flow. These are intriguing but small, and some use tasks with questionable real-world relevance.
The large long-term trial also included a cognition sub-study, and it did not find an overall benefit on the cognitive measures over several years. It’s possible flavanols help acutely without changing the long-term trajectory, or that the effect depends on baseline diet quality (one analysis suggested more benefit in people with poorer diets). Either way, “cocoa for brain health” is currently oversold relative to the data.
Insulin sensitivity, cholesterol, skin
Smaller bodies of evidence suggest modest improvements in insulin-sensitivity markers and small favorable shifts in cholesterol with cocoa flavanols, though results are inconsistent and many trials are short. A few studies found improvements in skin blood flow and hydration. All of this is preliminary.

Practical takeaways:
- 200 mg/day is the lowest dose with a regulatory-backed vascular claim and a reasonable floor.
- 500 mg/day matches the big outcomes trial and is a common supplement dose.
- Going much above ~900 mg hasn’t shown additional benefit in most work and raises the caffeine, theobromine, and contaminant considerations below.
- Timing doesn’t appear critical; the acute vascular effect peaks a couple of hours after a dose, so some people take it earlier in the day to avoid any stimulant effect on sleep.
Read labels for total flavanols or epicatechin content, not “cocoa extract” weight. A capsule of 400 mg cocoa extract could contain 20 mg of flavanols or 200 mg depending on standardization.
Safety and Interactions
Cocoa flavanols are well tolerated in trials, with side effects similar to placebo. The relevant cautions come from what travels with them:
- Caffeine and theobromine. Cocoa extracts contain both. Amounts vary by product (often 10–50 mg caffeine per dose), so count it against your total if you’re sensitive, have a heart-rhythm condition, or already take a stimulant-containing stack like caffeine and L-theanine.
- Blood pressure medication. The effect is small, but if you’re on antihypertensives, tell your prescriber — stacking several mild blood-pressure-lowering supplements with medication can add up.
- Heavy metals. Cocoa plants take up cadmium from soil, and some cocoa products contain enough that daily high-dose use adds meaningfully to exposure. Choose brands that publish third-party testing for heavy metals; this is one category where it genuinely matters.
- Migraine. Cocoa is a reported trigger for some people.
- Pregnancy and nursing. Normal dietary cocoa is fine; concentrated flavanol supplements haven’t been studied in pregnancy, so the default is to skip them unless your clinician says otherwise.
- Kidney function. Anyone with reduced kidney function should be cautious about high-dose cocoa products because of the cadmium issue above.
Where This Fits
