Pineapple season is a good excuse to look at one of the more interesting enzyme supplements on the market. Bromelain — extracted mostly from pineapple stems, not the fruit you eat — has been used clinically in parts of Europe for decades, which gives it a deeper research base than most “natural enzyme” products. It also gets sold for far more than the evidence supports.
Here’s where the research actually stands.
What bromelain is
Bromelain isn’t a single molecule. It’s a complex of protein-digesting (proteolytic) enzymes plus other compounds found in pineapple plants. Supplement-grade bromelain is standardized by enzyme activity — you’ll see units like GDU (gelatin digesting units) or MCU on labels — which matters more than raw milligrams. A common benchmark is around 2,000 GDU per gram.
One thing to get out of the way: eating pineapple is not a meaningful bromelain strategy. The fruit contains far less than the stem, and much of what’s there is degraded by processing and digestion. A reasonable question is whether a swallowed enzyme survives the stomach at all. Bromelain does appear to be absorbed intact in small amounts — studies have detected it in the bloodstream after oral doses — which is more than can be said for many enzyme supplements. Absorption is still limited, which is one reason effects tend to be modest.
Where the evidence is strongest
Sinus congestion and swelling
The most consistent bromelain research involves swelling and congestion — particularly sinus inflammation and recovery after dental or surgical procedures. Several controlled trials, some dating back decades, found that bromelain modestly reduced congestion, swelling, and discomfort compared with placebo, typically at 500–1,000 mg per day in divided doses.
In a few European countries, bromelain has actually been used as an adjunct in post-surgical care, which is unusual company for a supplement. The effect sizes are modest and some older trials wouldn’t meet modern standards, but the overall signal here is reasonably consistent.
Post-exercise soreness and minor swelling
A smaller body of research has tested bromelain (alone or in enzyme blends) for muscle soreness after hard exercise. Results are mixed — some trials found faster recovery of muscle function, others found nothing. If there’s an effect, it’s small. Where the evidence is mixed
Joint comfort
Bromelain shows up constantly in joint formulas, usually paired with curcumin. The human evidence is genuinely mixed: a few trials found bromelain-containing combinations comparable to low-dose conventional anti-inflammatory options for everyday joint discomfort, while other placebo-controlled trials found no significant difference.
The honest read: bromelain might help some people’s joint comfort at doses around 500–1,000 mg/day, but it’s a coin flip, and the better-designed trials tend toward smaller effects. It’s a reasonable experiment if better-supported options haven’t worked, not a first-line pick.
Digestion
As a protein-digesting enzyme taken with meals, bromelain does what you’d expect — it helps break down dietary protein. That’s useful mainly for people with genuine digestive insufficiency, and for that purpose a broad-spectrum digestive enzyme product usually makes more sense than bromelain alone. For everyone else, your pancreas already handles this fine.
Dosing and timing
Timing determines what bromelain does:
- For systemic effects (swelling, sinus, joints): take it on an empty stomach, at least 30–60 minutes before or two hours after eating. Typical studied doses: 500–1,000 mg/day, split into 2–3 doses. Check standardization — around 2,000 GDU/gram is a solid benchmark.
- For digestion: take it with meals, usually 200–500 mg.
Trials have generally run 1–2 weeks for swelling and up to 3–4 months for joint comfort. If nothing has changed by then, it’s not doing anything for you.
How to choose a product (if you’re trying it)
Bromelain labels are messier than most, so three things are worth checking:
- Enzyme activity, not just milligrams. Two “500 mg bromelain” products can differ several-fold in actual enzyme activity. Look for a stated GDU (or MCU) value — roughly 2,000 GDU per gram is a common quality benchmark, and a label that lists no activity units at all tells you something about the manufacturer.
- Enteric coating is a judgment call. Some products use enteric-coated capsules to shield the enzymes from stomach acid. The research base includes both coated and uncoated forms, so this isn’t make-or-break — but bromelain is somewhat acid-sensitive, and coating is a reasonable feature for systemic use.
- Watch out for blend dilution. Bromelain often appears in proprietary joint or enzyme blends at doses far below the 500–1,000 mg/day used in trials. As with any supplement, third-party testing seals (USP, NSF, Informed Choice) are worth more than any marketing claim on the front of the bottle.
Safety — this section matters more than usual
Bromelain is well tolerated for most people (occasional digestive upset and, rarely, allergic reactions), but it has a few genuinely important cautions:
- Blood thinners. Bromelain has antiplatelet activity and may amplify anticoagulant and antiplatelet medications — warfarin, clopidogrel, even regular aspirin. This combination is worth avoiding unless your prescriber explicitly okays it.
- Surgery. Because of the bleeding-risk angle, stop bromelain at least two weeks before any scheduled surgery.
- Antibiotics. Bromelain can increase absorption of certain antibiotics, including amoxicillin and some tetracyclines. That changes the effective dose of your medication — check with a pharmacist.
- Allergies. People allergic to pineapple should obviously avoid it, but cross-reactivity also occurs with latex, birch pollen, carrot, and celery allergies.
- Pregnancy and nursing. There’s insufficient safety data — skip it.
How it compares
Within the “natural anti-inflammatory” category, bromelain’s evidence sits below curcumin (more trials, more consistent results for joint comfort) but above most enzyme products, which have little systemic-absorption evidence at all. Its most distinctive, defensible niche is short-term swelling and sinus congestion — the applications with the longest clinical track record.
Bottom line
Bromelain is a legitimately researched enzyme with modest, believable evidence for sinus congestion and short-term swelling at 500–1,000 mg/day on an empty stomach, and mixed evidence for joint comfort. It is not a fat burner, a “natural antibiotic,” or a cancer therapy, whatever the marketing says. Its antiplatelet activity makes the blood-thinner interaction the single most important thing to know before trying it.
