Bilberry occupies an odd position in the supplement aisle. Almost everyone who sells it repeats the same origin story, that story is almost certainly false, and yet the actual research — which nobody markets, because it’s boring and hedged — is more defensible than the legend.
In January, when the mornings are dark and most of us are staring at backlit screens for the majority of our waking hours, it’s a reasonable thing to look at honestly.
The RAF story, and why it doesn’t hold
The legend goes like this: British pilots in the Second World War ate bilberry jam before night missions and reported dramatically improved night vision, which is how the berry’s power was discovered.
Historians of the period generally treat this as wartime misdirection — a cover story for radar, which was the actual reason British night interception improved and which nobody wanted to advertise. There’s no contemporaneous evidence of a bilberry program, and the story appears to have been retrofitted into supplement marketing decades later.
More importantly, the claim has been tested directly. Controlled studies through the 1990s and 2000s gave bilberry extract to people with normal vision and measured night adaptation and contrast sensitivity under low light. They generally found no meaningful improvement over placebo. A few small early studies suggested effects, but the better-controlled follow-ups did not confirm them.
This matters because “improves night vision” is still printed on bottles. It’s the weakest claim in the category, and it’s the one doing most of the selling.
There’s a real nutrient behind the confusion. Severe vitamin A deficiency genuinely does impair night vision, and correcting that deficiency restores it — that’s established nutritional science, and it’s about vitamin A, not bilberry. Anthocyanins are not vitamin A, and a well-nourished person does not gain night vision by adding either one.
What bilberry actually contains
Bilberry (Vaccinium myrtillus) is a European relative of the blueberry, with dark flesh throughout rather than the pale interior of a cultivated blueberry. The dark pigment is the point: bilberry is unusually rich in anthocyanins, a class of polyphenol that also gives elderberries, blackcurrants, and purple corn their color.

The proposed mechanisms are the usual polyphenol suspects: antioxidant activity, effects on the small blood vessels supplying the retina, and interaction with rhodopsin regeneration in the visual cycle. These are plausible in the laboratory. Anthocyanin bioavailability in humans is low — a small fraction of what you swallow appears in circulation as the parent compounds, with much of the activity likely coming from gut-derived metabolites — which is a real complication for translating cell studies into expected effects.
The modern research: screen fatigue, not night vision
The more interesting body of work looks at what researchers call VDT (visual display terminal) eye fatigue — the tired, dry, achy, hard-to-focus feeling after a long day on a monitor.
Several controlled trials, mostly conducted in Japan and Europe with office workers or students, have given bilberry extract at doses in the range of 160-480 mg/day over four to twelve weeks and measured subjective symptom scores alongside objective measures like accommodation and critical flicker fusion. A number of these reported improvements in self-reported eye fatigue, ocular discomfort, or shoulder and neck tension associated with prolonged screen work. Some also reported small changes in objective focusing measures.
The reasons to hold this loosely are substantial:
- Sample sizes are small. Many trials enroll a few dozen people.
- The primary outcomes are subjective. Eye fatigue questionnaires are legitimate instruments, but they’re susceptible to expectation effects in a way that measured acuity isn’t.
- Funding. Several trials come from extract manufacturers, which doesn’t invalidate them but does warrant a heavier prior.
- Replication is thin. These are not large multi-site trials, and independent replication of the eye-fatigue findings is limited.
So the honest summary is: a plausible but unconfirmed effect on how tired your eyes feel after screen work, with no credible evidence that bilberry improves how well you see.
Some research has also examined anthocyanins in the context of eye conditions involving the retinal microvasculature. Those studies are preliminary, conducted in clinical populations under medical supervision, and are not a basis for self-treatment — bilberry does not diagnose, treat, cure, or prevent any eye disease, and any persistent change in vision belongs with an optometrist or ophthalmologist rather than a supplement.
Bilberry is not a lutein substitute
This is the most common practical error. People shopping for “eye supplements” treat the whole shelf as interchangeable, and it isn’t.
Lutein and zeaxanthin are carotenoids that physically accumulate in the macula, forming the macular pigment that filters blue light. They have the stronger evidence base in the category, with large long-term trials behind their use in specific clinical contexts. Bilberry anthocyanins don’t accumulate in the macula and don’t do that job. If you’re taking one expecting the other’s benefits, you’ve bought the wrong thing.
They aren’t mutually exclusive — some formulas contain both — but the priority order is clear, and lutein has the better claim to your money.
Practical dosing and expectations
Dose. Studied ranges run 160-480 mg/day of extract standardized to 25-36% anthocyanosides. Higher isn’t better here; there’s no dose-response evidence supporting large amounts.
Timing. Usually split into two doses with meals. Polyphenol absorption is modest either way, and taking it with food improves tolerability.
Duration. Trials generally run four weeks minimum. If you’re going to test it on yourself, give it a month at a consistent dose and track one specific thing — end-of-day eye discomfort, for instance — rather than a vague sense of improvement.
What to expect. At best, a modest change in how your eyes feel after screen work. Anyone promising sharper vision is selling the myth.
The unglamorous interventions matter more than the capsule: deliberate breaks from close focus, blink awareness, adequate room lighting, correct screen distance, and an up-to-date prescription. Dry eye is the most common driver of screen-related discomfort, and it has actual treatments.
Safety
Bilberry fruit and standardized extracts are generally well tolerated, with mild GI upset the most frequent complaint. A few cautions genuinely matter:
- Blood thinners. Anthocyanins may have mild antiplatelet activity. Combining bilberry with warfarin, aspirin, clopidogrel, or other anticoagulants warrants a conversation with your prescriber. Discontinue at least two weeks before scheduled surgery.
- Blood sugar medication. Some evidence suggests anthocyanins may modestly affect glucose handling. If you’re on insulin or oral glucose-lowering drugs, monitor and discuss.
- Pregnancy and nursing. Data on concentrated extracts are insufficient; food amounts of berries are a different matter from a standardized extract, and the conservative answer applies.
- Bilberry leaf is a different product from the fruit extract, has been associated with toxicity at high or prolonged doses, and should be avoided.

Bottom line
The night-vision story that made bilberry famous doesn’t survive testing, and any product leading with it is telling you something about the seller. The real research — small trials on subjective screen-related eye fatigue at 160-480 mg/day of a standardized extract — is inconclusive rather than debunked, which is a meaningfully different position. If you want to try it for tired eyes after long screen days, it’s low-risk at studied doses provided you’re not on blood thinners. Just don’t buy it expecting to see better, and don’t buy it instead of lutein.
